A highly qualified nurse visits your home and makes an extensive assessment of your health, home environment, friend and family support.  Together you create a plan so you can relax and enjoy life safely at home. 

Sharing Information & Resources
During your meeting, our Nurse Care Manager will provide you with available and affordable resources within the community, and will assist you with health care options to support your independence.

Your Resthaven Care Manager sets up any services that you decide to use, and continues to check back with you and manage those who serve you.

Care Management 

How can a Resthaven Care Manager help you?

  • By performing an assessment in your own home to assure your safety and security.
  • By providing you with education regarding community resources, senior activities, support groups, and available meal programs.
  • By performing a comprehensive medication assessment and, if desired, speaking with your physician about any concerns.
  • By recommending reputable in-home assistance, matching your needs with community resources.
  • By reviewing financial, legal, and/or medical concerns and recommending specialists to assist you.
  • By acting as a liaison to your family, and alerting them if you experience any changes.
  • When needed, assisting you with the selection of a retirement home, assisted care home, or nursing home.
  • By providing information to meet your transportation and other mobility needs.
  • By making recommendations about modifications to your home that will make you safer and more comfortable.
  • Often services related to your health are covered by your insurance.  Your Nurse Care Manager is familiar with many insurances and will work with your physician and insurance company, when appropriate.
  • By continuing to check in with you to be certain that you are safe and happy with the services available to you.
